Supervisor Agreement Form

The Civil Rights & Title IX Policy at Southeastern Oklahoma State University requires that all employees with any supervisory responsibilities who witness conduct or receive complaints of discrimination or retaliation shall promptly report such actions to the EEO/Title IX Coordinator so that the University can ensure that complaints are promptly investigated. As a supervisor I agree:

To promptly report conduct I reasonably believe to be discriminatory or retaliatory.
To report discriminatory or retaliatory conduct of which I am aware, even if nobody has specifically complained about such conduct.
To report discriminatory or retaliatory conduct within ten (10) calendar days to the Equal Employment Opportunity Officer in the Office of Compliance & Safety.
To report discriminatory or retaliatory conduct even if it involves individuals who are not in my own chain of command at the University.
I understand that failure to report conduct I reasonably believe to be discriminatory or retaliatory can result in disciplinary action.
