A qualified student may apply for an extension of OPT while in a valid period of post-completion OPT. The extension will be for an additional 24 months, if all of the following requirements are met.<\/p>\n<h5>Requirements:<\/h5>\n<ul>\n<li>Currently be on post-completion 12-month OPT<\/li>\n<li>Have a degree in an eligible STEM field from a SEVP-certified school.<\/li>\n<li>Must have job offer from an employer registered with <a href=\"https:\/\/studyinthestates.dhs.gov\/understanding-e-verify\">E-verify<\/a>.<\/li>\n<li>Select a STEM OPT employer that provides the student with formal training and learning objectives directly related to field of study.<\/li>\n<li>Work a minimum of 20 hours per week per employer.<\/li>\n<\/ul>\n<h5>Applying or STEM OPT:<\/h5>\n<ul>\n<li>Completed Form I-765<\/li>\n<li>Completed Form G-1145<\/li>\n<li>I-765 filing fee: Money order\/cashier\u2019s check of $410 made out to \u2018US Department of Homeland Security\u2019<\/li>\n<li>Copy of Passport<\/li>\n<li>Copy of Visa<\/li>\n<li>Copy of your I-94<\/li>\n<li>Copies of OPT I-20s<\/li>\n<li>2 passport photos (write name and I-94 number on the back)<\/li>\n<li>Social Security card (if applicable)<\/li>\n<li>Copy of EAD card<\/li>\n<li>Copy of diploma or official transcripts<\/li>\n<li>Form I-983 Training Agreement<\/li>\n<li>Statement from employer\n<ul>\n<li>On letterhead explaining how current job\/position relates to your STEM major.<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>Mail to:<\/p>\n<h5>For U.S. Postal Services (USPS) Deliveries:<\/h5>\n<p>USCIS<\/p>\n<p>PO Box 660867<\/p>\n<p>Dallas, TX\u00a0 75266<\/p>\n<p>&nbsp;<\/p>\n<h5>For Express Mail Courier Deliveries:<\/h5>\n<p>USCIS<\/p>\n<p>Attn: NFB AOS<\/p>\n<p>2501 S.
